This female patient presented with a post traumatic deformity with septal deviation and airway obstruction from this as well as her vertically orientated tip cartilages . Overall, it made for an extremely difficult septorhinoplasty. The focus was on airway and shape , not size as making a small nose in the presence of such nasofacial disproportion is impossible as the excess is not just in bone and cartilage but also skin.
