This patient presented with septal deviation and a desire to address her nasal appearance and facial proportion. This was a particularly challenging problem because the skin was very thick and the patient desired quite a large change in her nasal profile. Limitations due to the thick skin were explained to the patient . In this case in order to achieve a result a second minor tip revision was required to help reduction of the thick skin envelope and allow for further tip de-projection.
